Which PDF are you referring to?

The most complete documentation for Ethereal development is generally in 
the README.developer and other README files in the doc folder of the source.

I'm by no means a core Ethereal developer but I've contributed a few 
patches to the developer docs and the patches have been readily accepted 
and committed.
